Issue: P13N: Using r3_env=intergration blocked by https sites 
Fix: When r3_env is being used in combination with a protocol-less rrserver address, use the sites location.protocol to decide on http vs https 

Issue: MVT List page as columns out of order 
Fix: Some columns are not always visible. That logic needed to keep better track of where they got inserted.
